Home Measured Health

Senior adults living with heart failure (HF) are provided with digital devices that measure blood pressure, glucose, oxygen saturation, and weight at home. The home-measured data is digitally recorded onto the digital platform

 

 

Remote Biodata Monitoring

A team of remote health professionals digitally monitors the user's blood pressure, glucose, weight, and oxygen saturation data. This monitoring, is meant to identify small  changes that point towards health deterioration. Biodata analytics triggers alerts.

 

Early Interventions

The early health deterioration alerts are sent to both the patient and the doctor for an appropriate intervention. Early intervention allows the patient to be advised and managed without hospitalization. This is a big economic saving and psychosocial benefit for the patient.
