Senior adults living with heart failure (HF) are provided with digital devices that measure blood pressure, glucose, oxygen saturation, and weight at home. The home-measured data is digitally recorded onto the digital platform.
Remote Biodata Monitoring
A team of remote health professionals digitally monitors the user's blood pressure, glucose, weight, and oxygen saturation data. This monitoring, is meant to identify small changes that point towards health deterioration. Biodata analytics triggers alerts.
